PDF in monochrome image

there also is an option

–ordered-dither NxN ordered dither the image

but it is not (yet) in the manual. Also I think the image is not yet converted to real Group4 encoding.

The image will than not realy be black and white, it will still be a color image, so the size will not change, and you can not use fax encoding.
